BAM Wellness Corporation D/B/A Clinical Hero shares personal identifiable information with entities who are directly associated with the clinical research study for which the potential research subject has consented to receive such information.
We make every reasonable attempt to safeguard subject privacy. Except where required by regulation,
court order, official authority, or applicable law Authorized, Clinical Hero (“CH”) will not
make individual information or data available to any other non-affiliated third party.
1. What kind of personal information does CH collect and hold? CH may collect
portions of your name, phone number, email address, postal code, date of birth, gender, height
and weight, race, or other information as required for a clinical research study. CH maintains
a registry of clinical trial participants to disseminate new trial information.
2. How does CH collect and hold this personal information? Interested participants register
themselves on the proprietary websites and receive an email. They can remove or edit the
information at anytime. All other information is encrypted in the CH database and password
protected.
3. Why does CH collect and disclose this information? In order to assist with recruitment of
a clinical research study, it is necessary for the study centre to quickly reach out to interested
motivated potential research subjects.
4. How can I contact CH to find out if my information is correct? How can I have
corrections made? You may inquire about your personal information in the database at any
point in time by contacting Clinical Hero via email at info@clinicalhero.com. If you are concerned that the
information in the Database is not accurate, you can contact CH at any time as directed
above. You will be required to confirm your identity. You may withdraw your personal identifiable
information (name, date of birth, email information) from the CH registry at any time without
penalty or loss of study benefits.
5. How can I make a complaint about a breach of the Canadian Privacy Principles, and how
CH will deal with such a complaint? You may make a complaint about a breach of the
Canadian Privacy Principles at any point in time by contacting Clinical Hero by email at info@clinicalhero.com. CH has internal policies and procedures in place to
investigate and resolve any breach of privacy. You will be notified of the outcome of the investigation
and any applicable actions taken as result of the investigation.
6. Is CH likely to disclose my personal information to overseas recipients? If so in what
countries are these recipients? Unauthorized people cannot access your personal
information; access to the registry is password protected. Only CH personnel will be able to
view your information in the database. CH employees in the United States will have access to
all your study information from all participating sites.
